Definition CKD Stage 4 CKD Stage 4 eGFR 15-29 mls/min Initial investigations Dipstick urine Send urine for Protein/Creatinine ratio if dipstick positive for protein Consider renal ultrasound if urological symptoms, or high risk of renal artery stenosis Consider diagnosis Monitoring Check eGFR and FBC every 3 months (recheck after 1 week if first abnormal eGFR) Treatment Treat underlying condition Modify lifestyle Treat hypertension Assess and treat other complications Educate patient Referral guidelines Refer to nephrology service unless clinically inappropriate
