Guidelines for the identification, evaluation and management of patients with Chronic Kidney Disease (CKD) The Derby 3-2-1 Project
Scope This guideline is designed to provide recommendations and guidance for the investigation and management of adult patients at risk of, or diagnosed with chronic kidney disease (CKD). It also covers objectives for the care and management of such patients, integrating the management of CKD with other vascular disease entities such as diabetes mellitus and ischaemic heart disease. The Renal National Service Framework outlines the importance of early detection and management of CKD, to both reduce the risk of progression to end stage renal failure for individuals, and to highlight CKD as a major risk factor for cardiovascular disease.
How to use this web site This web site is designed to give education and guidance for the diagnosis, assessment and monitoring of patients with chronic kidney disease (CKD). There are also pages covering particular complications of chronic kidney disease and referral to secondary care. This guidance cannot be comprehensive, so there are details for contacting local specialists and other web sites.
